Tennessee Athletic Commission

This is an application for an Initial Combatant, a Boxer, Mixed Martial Artist, Kickboxer, or Bare-Knuckle Boxer.

Requirements

Licensing Requirements

  1. Be at least eighteen (18) years of age.
  2. Color Photo (headshot)
  3. Photocopy of Driver’s License
  4. Blood test for HIV, Hepatitis B, and Hepatitis C must be taken within ninety (90) days of the application date.
  5. Medical Forms – The following items must be submitted on Tennessee-authorized forms found in the “Forms and Downloads” section of our website. Forms submitted from other states are not allowed.
    1. Neurological Exam completed within thirty (30) days of the application date for applicants age 35 or older.
    2. Eye Exam completed within ninety (90) days of the application date.
    3. Physical Exam completed within ninety (90) days of the application date.

Final processing of the application is subject to administrative review.

Application & Fees

Complete the “Initial Combatant Application” online at core.tn.gov. The fees associated with this application are $50 for the application fee and an additional $25 specialty fee.
